This form is to be used to:

  • Report issues with current Acumen Practice Notes
  • Identify content gaps and/or propose new Acumen notes
  • Propose changes to the Acumen website

Proposed new and updated content suggestions will be reviewed by the Acumen Content Review Panel (ACRP) and/or Environment review group.

Your Details

Current Note

Give the title of the Note you wish to report an issue with.
Copy and paste the Note's URL from the Acumen website.
Explain the issues that you have identified with the Note.

Proposed Note

If you wish to propose a new Acumen Note, give a detailed explanation of the content gap that you have identified and how you think it should be filled.
